What is LL-37?

LL-37 is a 37-amino acid cationic antimicrobial peptide — the only known human cathelicidin, derived from the C-terminus of the cathelicidin precursor protein hCAP18. It is a multifunctional innate immune effector that acts as an antibiotic, immune modulator, wound healing promoter, and anti-biofilm agent.

LL-37 is present in neutrophils, macrophages, mast cells, epithelial cells, and NK cells — broadly distributed across the innate immune system as a first-line defense against microbial invasion.

Mechanism of Action

Direct Antimicrobial Activity

LL-37 disrupts microbial membranes through electrostatic interactions with negatively charged bacterial, fungal, and viral membranes — inserting into and destabilizing the membrane structure. This mechanism is effective against gram-positive bacteria, gram-negative bacteria, fungi, and certain enveloped viruses.

Anti-Biofilm Properties

LL-37 is one of the most potent natural anti-biofilm agents identified — capable of penetrating and disrupting established biofilms that render chronic infections resistant to conventional antibiotics.

Immune Modulation

Beyond direct antimicrobial activity, LL-37 modulates immune cell function: recruiting monocytes and neutrophils to infection sites, activating dendritic cells, modulating macrophage polarization, and suppressing excessive inflammatory responses.

Wound Healing

LL-37 promotes wound healing through multiple mechanisms: stimulating keratinocyte migration and proliferation, promoting angiogenesis, and modulating the inflammatory phase of healing.
