ACE-031

What is ACE-031?

ACE-031 (Ramatercept) is a fusion protein consisting of the extracellular domain of Activin Receptor Type IIB (ActRIIB) combined with an IgG Fc domain. By acting as a 'ligand trap,' ACE-031 sequesters multiple myostatin superfamily members (myostatin, activin A, GDF-11, BMP-9) that inhibit muscle growth — dramatically elevating lean mass and bone density in research models.

Mechanism of Action

Myostatin and Activin Sequestration

Myostatin (GDF-8) is the primary negative regulator of muscle mass — the body's built-in brake on muscle growth. Activin A and related ligands provide additional inhibitory signals. ACE-031 binds these ligands with high affinity, preventing them from activating their cellular receptors and thereby releasing the brakes on muscle growth.

Muscle Hypertrophy

With myostatin signaling blocked, satellite cells become more active, myofiber size increases substantially, and muscle protein synthesis rates rise. Animal models lacking myostatin develop roughly double the normal muscle mass — demonstrating the magnitude of growth possible when this pathway is suppressed.

Bone Density Enhancement

The same ActRIIB pathway ligands that inhibit muscle also inhibit bone formation. ACE-031 therefore simultaneously increases bone mineral density alongside lean mass — relevant for conditions involving both muscle and bone loss.

Key Research Findings

Phase 2 clinical trials in Duchenne muscular dystrophy demonstrated significant lean mass increases within 3 months. Animal studies show dramatically greater muscle mass and bone density improvements versus any other known intervention. Phase 2 trials were paused due to side effects (telangiectasia, epistaxis) at higher doses — ongoing research examines dosing optimization.
